**The co-owners of Life Skills Resource Group, Cindy Fabico and Amy Smith have different fees than the rest of their practice due to their years of experience, depth of knowledge, and the demand for their services.

Regarding the other therapists who work at Life Skills Resource Group, not every therapist has availability at $60 fee but we always have at least a few who are taking new individuals of all ages as well as couples and families as it is important to us that we can service the entire community. If you feel that even the lowest sliding scale fee of $60 per session will create a significant financial hardship, please speak to your therapist or life coach about this during your free phone consultation. They may be able to arrange for a lower fee to offset the cost or schedule less frequent sessions to fit within your budget.
Cash, check and all major credit cards accepted for payment.
We have a 24-hour cancellation policy. If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance by phone (no email cancellations please), you will be required to pay your session fee.
We are primarily a self-pay practice, but we are able to assist with your insurance. We are happy to check with your insurance company about your out-of-network mental health benefits. If you would like us to contact your insurance company on your behalf, please complete this Insurance Verification Form and we will contact your insurance company. We will not seek any pre-authorization without your approval. We promise to get back to you within 24 hours to discuss how your benefit works at our office (on Monday for forms submitted Friday and Saturday).
Please note that although we are happy to assist with your insurance, all information provided to us by insurance companies is based on current information and actual coverage information may prove to be different when a detailed Explanation of Benefits is received with the insurance payment. We also cannot accept responsibility for collecting or for negotiating a settlement of a disputed claim. You are responsible for any balance that your insurance carrier does not pay.
An important note about insurance coverage
Insurance payment for mental health visits is based on medical necessity, which means that you must have a “diagnosable” disorder and your counselor must provide information about the services provided to you to your insurance company. This is true for all types of counseling. If you want to use your insurance, your counselor will be assessing during your first visit if you have a “diagnosable” disorder. If this is the case, we will be happy to help you get your counseling covered, but be aware that you will be diagnosed and that this diagnosis becomes a permanent part of your medical record.
Please also be aware that in most, but not all, cases marriage or relationship counseling is not a covered insurance benefit. As above, your counselor will be assessing during the first visit if one or both of the spouses has a “diagnosable” disorder that would allow insurance coverage.
Life Coaching is not covered by insurance.
Life Skills Resource Group, LLC reserves the right to refuse service to any potential client, based on the American Counseling Association (ACA) Code of Ethics 2005.
