Sliding Scale Fees
Based on Household income.  We do not require documentation.  We are on the honor system.  During your free consultation if the standard fee is not appropriate for you please discuss this with your therapist.

Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance by phone (no email cancellations please), you will be required to pay the full cost of the session as booked.

Insurance
We are a self pay practice but this does not mean we do not take insurance. We choose not to be "preferred providers" for any insurance plans due to the restrictions and detailed reporting of your confidential information required.   Most PPO and many HMO plans have provisions for "Out of Network" Mental Health Benefits. We encourage you to contact your insurance company and ask these questions:

  Do I have "Out of Network" Mental Health Benefits?
  How much is my deductible?
  How much of the deductible has been met?
  What is the maximum number of psychotherapy/counseling visits per year? (typical out of network benefit is 20 per year per family member)
  What is the maximum amount that can be paid per year?
  Is there any preauthorization or referral required?  If so who must provide the referral?
  What address should I mail claims to?  Can out of network providers bill directly (we will do this if your insurance allow it)?  Is it the same address?
  Is there a special form that claims need to be sent in on?  Is it the HCFA 1500?
  What is the fee schedule for the following CPT codes in zip code 32819 for a Master's level provider:
90801-Initial Assessment Visit
90806-50 minute follow up visit
90808-75 minute follow up visit
  What percentage of fee schedule do you reimburse for out of network visits?

Most "out of network" benefits will pay 50-100% of the "usual and customary charges" and if you come to us with this information we can work with you so that your out of pocket cost nothing or minimal.  If your insurance company will allow us to bill out of network most of our therapist are willing to accept the same copay you would give your in-network provider and bill your insurance the balance.
