SLIDING SCALE FEES
Based on Household income. We do not require documentation. We are on the honor system. During your free consultation if the standard fee is not appropriate for you please discuss this with your therapist.

PAYMENT
Cash, check and all major credit cards accepted for payment.

CANCELLATION POLICY
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance by phone (no email cancellations please), you will be required to pay the full cost of the session as booked.

FULL AND PARTIAL SCHOLARSHIPS
Are available- if you feel that even the lowest sliding scale fee of $60 per session will create a significant financial hardship, please speak to your therapist or life coach about this during your free phone consultation. They may be able to arrange for a lower fee using our scholarship fund to offset the cost or schedule less frequent session to fit within your budget.

We are primarily a SELF PAY practice but this does not mean we do not take insurance. We are happy to check with your insurance company about your Out of Network Mental Health Benefit if you will complete this Insurance Verification Form we will contact your insurance company on your behalf. We will not seek any pre-authorization without your approval. We promise to get back to you within 24 hours to discuss how your benefit works at our office (on Monday for forms submitted Friday and Saturday).

An important note about insurance coverage for Marriage or Relationship Counseling:

In most but not all cases Marriage or Relationship Counseling is not a covered insurance benefit. Insurance requires a Mental Health Diagnosis in order to cover counseling. If you are seeking Marriage or Relationship Counseling and want to use your insurance your counselor will be assessing in the first visit if one or both spouses has a “diagnosable” disorder and if this is the case we will be happy to help you get your counseling covered but be aware that you will be diagnosed and that this diagnosis becomes a permanent part of your medical record.

Disney Medical Plan Participants

Effective 1/1/2010 Disney Cast members will be covered by Cigna Behavioral Health and Cindy Fabico, Angie Moates, Maribel Nieves, and Jean Austin-Danner of Life Skills Resource Group are In Network Provider for this Insurance Company.  You DO NOT need to complete the Insurance Verification form but you DO need to call the CIGNA/EAP/BH/SA # on the back of your insurance card 1-800-952-6676 to get an EAP approval code.  This code will allow ANY Disney Cast Member (full-time, part-time, contract, seasonal), as well as any family member covered under the benefit plan, 5 FREE EAP session per year.  Once you have used your 5 sessions, you have unlimited sessions with a $15 copay.  We will expect you to have this CODE and your insurance information ready at your first session or we will expect you are prepared to pay the $15 copay.
